CPE Prior Approval Instructions

Obtaining Prior Approval for Programs (CPEs – continuing professional education)

• Go to the website  http://www.cdrnet.org/pdrcenter/prior.htm and print off the Continuing Education Prior Approval Request Form. 

• Complete the form according to the instructions and send with all documents requested (see instructions on web page). 

• The form, documentation and fee should be sent to the MDA office at 4940 Viking Dr Ste 388, Edina, MN  55435 at least a month prior to the event. 

• Fees for processing are:
     o 1-2 clock hours  $25
     o 3-4 hours   $50
     o 5 hours to one day  $100
     o each additional day  add $50
